Request deletion of your personal data from Formiva.
Your Right to Erasure
Under data protection regulations including GDPR, you have the right to request the deletion of your personal data. We are committed to honoring these requests promptly.
Note: Uninstalling the Formiva app from your Shopify store does not automatically delete your data. To ensure complete data removal, please submit a deletion request.
What Data Will Be Deleted
Upon receiving your deletion request, we will remove:
- Your store information and account data
- All forms you have created
- All form submissions and responses
- Analytics and usage data associated with your account
- Any other personal information we have collected
Warning: Data deletion is permanent and cannot be undone. Please ensure you have exported any data you wish to keep before submitting a deletion request.
How to Request Data Deletion
Send an email request
Email us at [email protected] with the subject line "Data Deletion Request".
Include your store information
Provide your Shopify store URL (e.g., your-store.myshopify.com) and the email address associated with your account.
Verification
We may contact you to verify your identity before processing the request.
Confirmation
Once your data has been deleted, we will send you a confirmation email.

We will process your deletion request within 30 days. In some cases, we may need additional time, in which case we will notify you of the delay and the reason for it.
